Remember to also to complete and turn in the Contestant Contract (signed and notarized), copy
of your birth certificate and driver's license, Scholarship rules and regulations (signed and notarized), Newspaper information
form, and your talent tape (if you require one).
Also, photos are due at the first rehearsal along with your paperwork. You are required
to submit SEVEN (7), 5x7, black & white photos that will be used for the Judges' Books, Program
Books and newspaper. Failure to adhere to these guidelines could result in your photo not being included.
